Youth Ministry E-VINE Announcement (January 23, 2014)

Updated Forms and Procedures from the Legal Counsel

-- Student & Youth Activity Form and Medication Authorization & Permission Form
Over the past few months, our office in collaboration with other Archdiocesan offices worked with the Legal Counsel to revise permission slips and medical release forms with the goal of drafting a universal form that could be used by parishes and schools. The revised forms are worded to reflect changes in law and the language should not be changed when using them. The updated forms need to replace your existing forms. The Student and Youth Activity Form is both a permission slip and medical release form. When completing the form make sure the LOCATION states the sponsoring Archdiocesan school, parish or department who is taking responsibility for the young person during the event. For instance, when bringing youth to Youth Day the name of your parish/school should be listed as the LOCATION because you are assuming responsibility for the participants from your parish/school. The Medication Authorization & Permission Form should be used when a young person needs to be given prescribed medication and/or a young person needs to carry and use emergency medication (i.e. inhaler, EpiPen, insulin etc.). The forms are updated on the Office of Youth Ministry website

Youth Day and Congress
Today, January 23 is the last day to register for Youth Day and Congress at the discounted rate. The fee increase is as follows: Youth Day to $35 per person; and Congress to $80 per person.

Youth Day 2015 Theme

Last month about 35 young people and adults gathered to read the readings of the day for Youth Day 2015 and came up with a great theme. Our 2015 theme will be “Talk Jesus to Me”! A special thanks to the team that gathered to do the hard work of praying over the readings and discerning a theme! We used the reading from the Thursday of the Third Week of Lent. We hope you will help get the word out about our great day – Thursday, March 12, 2015 – and we encourage you to use the readings and theme as you prepare for that day. More information will come out this fall!
